Your personal information
Haley Edwards- Counselling & Psychotherapy obtains and processes your personal information for the purpose of offering a counselling service.
The personal information collected for the service includes:
• Personal contact details- name, address, phone number, email address, date of birth
• Emergency contact details- named emergency contact, GP details
Personal information will be used to communicate with you and maintain records of our counselling sessions. Communication will be via phone, text and email.
Notes of session content will be coded to protect and respect your confidentiality. These notes will capture only factual information and main themes of the session. Session notes will be password protected and retained electronically for a period of 6 years before being destroyed. They will be used for counsellor recollection, supervision of practice and lawful obligation.
In line with BACP ethical guidance, all counsellors are required to have clinical supervision by an external supervisor. Privacy and anonymity will be respected and protected in any case discussion.
Personal information retained in paper format will be stored in a locked facility and all electronic records will be coded and password protected. All personal data will be held for 6 years post therapy end date in line with insurance and ICO recommendations. After this date, all records will be securely destroyed.
Personal information and data is not used for marketing.
Safeguarding & Confidentiality
Personal information is obtained and stored securely to enable the counselling service in line with the agreed safeguarding processes discussed in our counselling contract. Your personal information will not be shared without consent however, there are some lawful exceptions where confidentiality may need to be breached. In these exceptional circumstances Haley Edwards- Counselling and Psychotherapy will discuss this with you unless prohibited.

Your rights
As set out by UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018 (DPA 2018), you have the following rights regarding the data that we hold about you:
• Right to access the information held about you- you can submit a data access request which will be addressed within 1 month of receipt.
• Right to rectification- you can ask us to correct any factual mistakes we hold about you or request that information is completed where it hasn’t been.
• Right to erasure- you can request to have the data we hold about you deleted in certain circumstances.
• Your right to opt out of your data being processed- you can ask us to restrict the processing of your personal information in certain circumstances.
• Your right to object to your personal data being collected.
• Your right to data portability- you can ask that we transfer your personal information for you to use for your own purposes across different services in certain circumstances.
